I conducted discussions with four users with various chronic health conditions to grasp a comprehensive understanding of users' needs from an application that supports them in managing their progress. I learned about their practices for data input and monitoring, doctor collaboration, and personalized journey creation, along with the difficulties they encountered in building personalized patient plans.

To address this, we gave users the option to manually edit their milestones directly and introduced an additional feature in which they could converse milestone modifications with the chatbot.
A few users expressed concerns and hesitancy towards divulging their personal health goals and medical data to our AI tool. To address this, we opted to personify our AI assistant by naming it 'Coco', and attaching a welcoming avatar for interactions with users.
